New Patient Information

Appointments can always be scheduled within one week. Please plan to arrive 15 minutes ahead of your scheduled appointment to allow time for registration. If we have your email address, you will receive an email invitation to join our patient portal powered by Navigating Care. On the portal website, you will create an account and proceed through the registration wizard. Please be as thorough as possible when entering your medical history, and list any medications you are currently taking. By doing so, you will greatly reduce the amount of time spent in registration at the office. Bring your insurance cards, photo ID, and a list of your medications. If you have questions, we suggest you write them down prior to your appointment. The first visit to our clinic is very informative and there will be a great deal of information presented to you. Writing down questions beforehand will ensure you are as prepared for your visit as possible. Your doctor will review all of your results, and together, you will formulate a plan of care.

Diagnostic Studies & Imaging

Your doctor may find it necessary to order diagnostic studies or other imaging. A member of our staff will schedule any imaging studies or procedures at the appropriate hospital or agency and in the time frame ordered by your physician. Except for emergency situations, diagnostic studies, imaging or other procedures will not be scheduled while you are in the office. If your insurance company requires that we obtain prior authorization for these studies or procedures, we will obtain that authorization on your behalf prior to your scheduled appointment. Our scheduling staff will contact you by telephone or mail the documentation to you informing you of your appointment as well as any special instructions pertinent to the scheduled event. Should you have questions about diagnostic scheduling or insurance pre-authorization, please contact our office and we will be happy to assist you.
